Insurance Information
Bring with you to your appointment:
- Face mask or scarf to cover your nose and mouth
- Current insurance card
- List of current medications and medical conditions
- Primary-care physician address and phone number and contact information for any physicians involved with your care
- Recent diagnostic images such as CTs, MRIs, x-rays, bone scans, or EMGs
Expedite your check-in: Patients who are new to ONS or who have not visited our offices in more than a year should complete the patient registration form on the ONS Patient Portal. Patients that do not register on the patient portal, must arrive 20 minutes before their appointment time to complete in office.
