Recent diagnostic images such as CTs, MRIs, x-rays, bone scans, or EMGs
Patients who are new to ONS or who have not visited our offices in more than a year should complete the patient registration form on the ONS Patient Portal. If you do not pre-register on the patient portal, you will need to arrive 20 minutes before your appointment time to complete paperwork. This will expedite your check-in.
