New Patients or patients who have not visited ONS in more than a year, please arrive 15 minutes prior to your scheduled appointment time to complete your registration or enter the Patient Postal to complete ahead of time. Also please also bring the following to your appointment:
- Insurance card
- List of current medications and medical conditions
- Primary-care physician address and phone, and contact information for other physicians involved with your care
- Recent diagnostic images such as CTs, MRIs, x-rays, bone scans, or EMGs. Bring both original films and copies of reports.
DIRECTIONS TO ONS in STAMFORD
From Downtown Stamford
- Go north on Atlantic Street toward Stamford Forum. Atlantic becomes Bedford St.
- Turn right onto High Ridge Rd.
- Turn right onto Buxton Farm Rd., then turn left onto Turn of River Rd.
- Turn of River Rd. becomes High Ridge Park.
- High Ridge Park is on the right. Building 5 is the 2nd building on the left.
- Park on the lower level in the back of the building. ONS is on the 3rd floor.
Northbound
- CT-15/Merritt Pkwy: Exit 35 (CT-137/High Ridge Rd.), go straight at the light at the end of the exit ramp then continue on Buxton Farm Rd.
- In .7 miles, turn left onto Turn of River Rd. and continue into High Ridge Park.
- Building 5 is the 2nd building on the left.
- Park on the lower level in the back of the building. ONS is on the 3rd floor.
Southbound
- CT-15/Merritt Pkwy: Take Exit 35 (CT-137/High Ridge Rd.), at end of the ramp, turn right onto High Ridge Road (CT-137).
- In .1 miles, turn left onto Buxton Farm Rd.
- In .7 miles, turn left onto Turn of River Rd. and continue into High Ridge Park.
- Building 5 is the 2nd building on the left.
- Park on the lower level in the back of the building. ONS is on the 3rd floor.
Patient Drop off and Parking
Patient-drop off entrance and handicap accessible parking in on the lower level. As you enter the driveway to building 5, watch for the patient drop off sign on the right. Handicap parking and other patient parking is on lower level. ONS and ONS Physical Therapy are on the 3rd floor.
